Thank you for visiting the How to Order page for the Books and Videotapes section of the Teacher's Workshop website. This page will assist you with orders for books or videotapes.

Our order form for books/videotapes is set up for printing (for credit card purchases see below) and is offered in three different formats. For your convenience, you may access the order form in either html, pdf, or rtf format:

	- View and print order form as a web page
	- (Portable Document Format) View and print with Acrobat Reader (Acrobat Reader is a free program that works with your web browser. If you don't have Acrobat Reader installed, you may want to download and install it now.)
	- (Rich Text Format) View and print from any recent word processing program

CREDIT CARD PURCHASES: The great majority of our customers purchase with a check or with a copy of their purchase order submitted along with the order form. We are also happy to accept your VISA or Mastercard purchase. However, we cannot accept your credit card from this section of our website. Please add your credit card number and expiration date to the completed order form and fax to us at (706) 769-4137; or you may prefer to call us at (800) 991-1114.
